Simple implementation of a PE6490x digitally controlled capacitor in a shunt configuration with one fixed parallel component and two fixed series components. The simplest use would probably be as a bandpass filter with an inductor in Z2 and 0 ohm jumpers in Z1 and Z3. C1 is for decoupling. Passives are 0603.
