Found this schematic for an ANSI Type-2 Opto-coupler on www.solarshare.net, licensed under Creative Commons Attribution 3.0 Unported License. I had to create the board outline file using the procedure documented by OSH Park using gerbv, and the drill file had to have the file extension changed to .XLN and the first two lines had to be swapped (the M48 must come first). Currently untested.
